CHECK-IN REQUREMENTS

Team registration requirements are as follows:

1. Five (5) copies of roster with uniform numbers listed by each player name
2. Laminated Player Cards and Coach cards with pictures
3. Medical release forms
4. Out of state travel permission form
5. Parent Liability Release Form  ( Download  here -->) OpenDocument
6. Registration will take place at the Richard Siegel Championship Soccer Complex main concession / meeting area at the following times: 

1. Friday, Oct. 19 - 4:30PM - 8:00PM
2. Saturday, Oct. 20 - 7:00AM - 2:00PM

You must be registered at least one 1 hour prior to your first game.
