Team Check In –
All teams must check-in using the online check in. You can complete that by clicking on the Online Check-In button on the left of the the page.
Online check-in must be completed by Wednesday, February 8 by 4pm! No exceptions!!
All teams must submit:
- USYS or US Club Match and four (4) copies of match rosters
- Guest Player Roster (if applicable)
- Player medical consent Forms
- Individual player/member passes/cards
- Any out-of-state teams, an approved “notification to travel” form
Guest Players
- You may have 5 guest players with any 11U – 14U team.
- Must have 5 copies of the Guest Player Roster
- Must have the Guest Player's player card
- Must have the Guest Player's Medical Consent Form - made out to the team attending the tournament
