Document | Medical Release for PlayersAll teams are required to have one medical release form per player regardless of your club or association rules. This is a requirement to play in Beat the Heat. Forms will be checked, make sure that they are signed. An unsigned form will cause a player to be removed from your roster.

This is used for US Club guest players that are outside of your club.
If you have a guest player from INSIDE your club, you will need to use the EVENT'S guest player form. This form is also found in the Support Files of the Online Check-in.
