• GA Soccer official roster (guest players up to three should be written in)
• Laminated player cards with photos for each player
• Tournament Release Waiver (signed by a parent of each player)
• Tournament Covid Waiver (signed by a parent of each player)
• Five (5) printed game cards from the tournament/Soccer in College system (with accurate roster)
• Medical Release form - will not upload, but have with you at all times.
• Payment in full
