Open Document	Medical Release Form Each registered/accepted team for the Secret City Fall Tournament is required by the City of Oak Ridge to submit a completed Medical Release form at time of team check in. This form MUST be filled out and signed by each players parent or legal guardian.
Open Document	Assumption of the Risk and Waiver and Release of Liability Relating to Coronavirus/COVID-19 Each registered/accepted team for the Secret City Fall Tournament is required, by the Secret City Fall Tournament to, submit a completed Covid Waiver form at time of team check in. This form MUST be filled out and signed by each players parent or legal guardian, uploaded to the OnLine Check-in, or completed online within the OnLine Check-in.
